Diabetes prevention and management in general practice: Using the Pen Computer Systems Clinical Audit Tool
The PCS Clinical Audit Tool (CAT) is a data extraction tool for general practice that makes systematic diabetes prevention and management easier. The CAT allows you to quickly and effortlessly look at a snapshot of your data to get valuable practice information such as:
- Patients at high risk of type 2 diabetes
- Number (and names) of patients with diabetes in your practice
- Patients who are at target levels for HbA1c, blood pressure and cholesterol
- Diabetes SIP items remaining
Diabetes Australia – Vic has produced this evidence-based resource to guide you through step by step instructions for using the CAT for effective prevention and management of diabetes. There are screen shots for easy comparison with your own software, activities to practice your new skills and plenty of spaces to take notes as you work through the sections.
