The General Practice Program supports general practice to undertake systematic prevention and management of diabetes.

Free information and support for your patient with diabetes

When you diagnose a patient with diabetes please ensure that you tick YES to "I would like you to receive information about the benefits of membership of Diabetes Australia on their National Diabetes Services Scheme (NDSS)" registration form or tell them to phone 1300 136 588 to become a member.

All newly diagnosed people get a FREE 12 month membership to DA–Vic.

  • Diabetes Today and Issues magazines with the latest news on diabetes issues
  • A 10 percent discount on Diabetes Australia products at 60 locations around Victoria

Managing your patients

We have a range of programs and resources available to assist in the diagnosis and management of your patients with diabetes or phone 1300 136 588.

We strongly encourage you to recommend your patients become a member of the National Diabetes Services Scheme (NDSS).  The NDSS is administered by DA–Vic and provides subsidised products for people to control their condition as well as education and training on how to best self-manage key elements such as personal diet and exercise. Products such as blood glucose testing strips, urine testing strips and insulin pump consumables are available at greatly reduced prices to registrants.

For your patients

Your patient can contact DA–Vic on 1300 136 588 to receive a range of diabetes prevention and management information, multilingual advice, advocacy, access to the NDSS program and locations of PARS pharmacists, locations of supermarket tours and access to the Dose Adjustment For Normal Eating (DAFNE) program.

For multilingual patients

We are committed to providing vital information and support to our culturally and linguistically diverse communities (CALD). If you have CALD patients asking about diabetes prevention or management, you can refer them to the our Multilingual Infoline telephone numbers which connects the caller with an interpreter and a DA–Vic health professional (diabetes educator, dietitian and/or advocacy officer). This service is free and easy to use.
