The Life! program offers free health assessments to people found at to be at high risk of developing diabetes. A health professional will meet you in person or call you for a one-on-one discussion about your risk of developing type 2 diabetes.
Free health assessments are available to people who do not have diabetes and meet one of the below criteria:
- Aged 50 years or older and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool; OR
- Aged 18 years or older, of Aboriginal or Torres Strait Islander decent and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool; OR
- Aged 18 years or older and previously diagnosed with heart disease or diabetes during pregnancy (gestational diabetes); OR
- Aged 18–39 years, have had a WorkHealth assessment and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool.

See your doctor
This is especially important if you haven’t seen a doctor or discussed your diabetes risk for a while.
Discuss your Australian Type 2 Diabetes Risk Assessment tool score with your doctor and ask to have a fasting blood glucose test to make sure you do not already have diabetes. People at high risk should complete this test every 12 months. If your results come back abnormal, your doctor may order a glucose tolerance test (like a stress test for diabetes) on another day.
If you do not have diabetes ask your doctor to refer you to the Life! Taking Action on Diabetes (Life!) program by completing the referral form and faxing it to (03) 9667 1757. You may also be eligible for a free health assessment. Call 13 RISK (13 7475) for further information or to organise this.
