Improving the Diabetes Journey
IEPCP in conjunction with the OEHCSA is implementing the project entitled ‘Improving the Diabetes Journey’. The Improving the Diabetes Journey project is a systems-level change project, which supports local partnering agencies to enact change to improve care for people with chronic disease (with a specific focus on type 2 diabetes). The aim of the project is to improve the health and wellbeing of people with or at risk of type 2 diabetes, through the development of a regional approach to the prevention and management of type 2 diabetes.
Project principles
Improving the Journey is built on the following key principles:
- Consumers at the centre
- Equity of access
- Focus on populations at risk
- Evidence based systems and services, and;
- A whole of system (redesign) approach.
Project outcomes
Predicted outcomes over 4 year project:
- Improved consumer experience of coordinated care
- Improved processes to support prevention, early identification and intervention for people at risk of type 2 diabetes and its complications
- Improved awareness of services available, by consumers and clinicians
- Development of processes which support coordinated care across services and disease stages
- Increase in integrated processes for prevention and management
Framework
A framework for service delivery for people with type 2 diabetes in the Eastern Metropolitan Region (EMR) has been developed. The framework outlines the services that all consumers with type 2 diabetes in the EMR must have the opportunity to access, at different life or disease stages. This framework is referred to as the ‘Type 2 Diabetes Care Pathway’.
2012 Project
The 2012 ItDJ project will continue to focus on system change and aims to support agencies to plan and implement changes to support the delivery and use of the:
- Type 2 Diabetes Care Pathway
- The Type 2 Diabetes Priority Pathway Tool
- The Client Information Pack Recall and Reminder Map, and
- additionally a GP Communication Plan.
In 2012 there will be quarterly meetings. The meetings will be facilitated and will involve joint planning, problem solving and periodic learning sessions.
It is recommended agencies continue to use the quality improvement framework of ‘Plan, Do, Study, Act’ to identify small part(s) of the ItDJ tools and processes to implement within your organisations. It will be expected that agencies submit 2 PDSA Reports every quarter (a total of 7).
