Life! Taking Action on Diabetes (Life!) program
The Life! Taking Action on Diabetes (Life!) program aims to prevent type 2 diabetes in Victorians identified as eligible and at high risk. The Life! course helps participants set small, achievable goals to improve their lifestyle and reduce their risk of type 2 diabetes.
Key to the Life! program is the support participants receive from trained health professionals, including a dietitian and a physiotherapist/exercise physiologist, to improve their nutrition and increase physical activity with the aim of reducing their weight and waist circumference.
The Life! program is delivered in two modes - the group-based Life! course and individual Life! telephone health coaching sessions.
The six session course takes place over eight months to allow participants to put their new skills into practice. The first five sessions take place fortnightly, with the last session taking place eight months after the first.
Eligibility
The Life! program is available to people who do not have diabetes and meet one of the below criteria:
- Aged 50 years or older and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool; OR
- Aged 18 years or older, of Aboriginal or Torres Strait Islander decent and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool; OR
- Aged 18 years of older and previously diagnosed with heart disease or diabetes during pregnancy (gestational diabetes); OR
- Aged 18–39 years, have had a WorkHealth assessment and scored 12 or more on the Australian Type 2 Diabetes Risk Assessment tool.
