Membership
Diabetes Victoria (ABN 71 005 239 510) is a registered charity by the Australian Charities and Not-For-Profit Commission.
By becoming a member of Diabetes Victoria, you are agreeing to the following terms and conditions.
Diabetes Victoria is committed to protecting the personal information provided by you. By submitting this form, you acknowledge and consent for your contact details to be collected by Diabetes Victoria to communicate with you about your membership and future opportunities to support Diabetes Victoria.
Membership terms and conditions
Diabetes Victoria offers a range of membership options, designed for people living with any type of diabetes. We also offer memberships for health professionals.
Memberships are available for individuals and families only.
To be eligible for a concession membership you must be aged 25 years and under or 65 years and over, and/or hold and be able to provide details of your Australian Pensioner Concession care, Healthcare Card, Disability Concession or DVA Gold and White Card, or full-time Student Card at an Australian school or tertiary institution.
Children and young people (aged 21 years or younger) who live with diabetes are eligible for free Kids and Teens membership.
Life memberships, and any other free memberships, are by invitation only. These are offered at the sole discretion of Diabetes Victoria.
Memberships are not transferable and must only be used by the named person or persons on the membership card.
Diabetes Victoria may refuse any application for membership at its discretion. Diabetes Victoria may change or revoke any membership options at its discretion.
Diabetes Victoria will continue to keep past members informed about membership offers and information unless a lapsed member has unsubscribed from communications.

Membership benefits
Membership grants the listed member or members access to benefits including a personalised membership card, a subscription to our seasonal membership magazine, access to rewards and discounts, and a subscription to our monthly membership e-newsletter.
These benefits are provided for the duration of your membership.
Membership benefits may vary depending on your membership option.
Membership benefits may change from time to time. New benefits or changes to membership benefits, are communicated to members via the website.
Diabetes Victoria is not liable for any change to, or difference in, membership options or membership benefits.
For kids and teens members, some of the benefits available to our members are intended for a parent/guardian of someone living with diabetes and/or an adult living with, or affected by, diabetes.
You must undertake your own inquiries to determine the relevance of any membership benefits, offers or other information.
Cards we accept
We currently accept Visa, Mastercard and American Express credit cards for online payments. Payments are made via a secure payments system.
Refunds and cancellations
Membership refund requests are only accepted in writing, hard copy or email.
Memberships purchased through Diabetes Victoria have a 21-day cooling off period, any request for refund after the cooling off period will not be accepted.
In the instance where a member has been incorrectly charged, a refund can be processed.
Members who have accidentally overpaid or paid for their membership more than once in one year are also eligible for a refund.
Except to the extent required by law, memberships are not exchangeable and are not refundable on any other basis.
Membership refund requests on a pro-rata basis will not be considered.

Membership renewal
Diabetes Victoria will contact active members before their membership expiry, to provide information about their membership renewal.
Membership renewal information may be sent by direct mail, email, and telephone.

Automatic membership renewal terms and conditions
Definitions
account means the account held at your financial institution from which we are authorised to arrange for funds to be debited.
agreement means these terms and conditions agreed between you and us.
banking day means a day other than a Saturday or a Sunday or a public holiday listed throughout Australia.
debit day means the day that membership payment by you to us is due. This is your membership renewal date.
debit payment means a particular transaction where a debit is made.
direct debit request means the automatic membership renewal request between us and you.
annual membership fee means your membership payment amount. This is the fee associated with your membership option.
us or we means Diabetes Victoria, (the Debit User) you have authorised by requesting a Direct Debit Request.
you means the customer who has consented to, or authorised by other means, the Direct Debit Request.
your financial institution means the financial institution nominated by you on the DDR at which the account is maintained.
1. Debiting your account
1.1 By opting into an automatic membership renewal, you authorise us to arrange for your annual membership fee to be debited from your account on your debit day. You should refer to the Direct Debit Request and this agreement for the terms of the arrangement between us and you.
1.2 We will arrange for your annual membership fee to be deducted in one payment on the date of joining and then annually on your debit day under these terms and conditions. This includes consent to multiple debit attempts if a payment fails to transfer.
1.3 Diabetes Victoria will endeavour to contact you by email before your debit day to remind you of your upcoming debit day. It is your responsibility to ensure the contact email address you have provided is correct and up to date.
1.4 It is your responsibility to ensure the account details you have provided are correct and up to date, including letting Diabetes Victoria know if the expiry date for your nominated credit or debit card changes or changes to an account.
1.5 You must have sufficient cleared funds available in your nominated account on the scheduled debit day.
1.6 If the debit day falls on a day that is not a banking day, we may direct your financial institution to debit your account on the following banking day. If you are unsure about which day your account has or will be debited, you can ask your financial institution.
1.7 If your debit payment is unsuccessful or dishonoured, we will contact you to request payment.
1.8 Your renewed membership, and access to membership benefits, will not be confirmed until a successful payment has been processed.
1.9 You agree to the automatic renewal of your annual membership with Diabetes Victoria from year to year unless you choose to opt out by emailing membership@diabetesvic.org.au or by telephoning us on 1300 437 386 during business hours by no later than five (5) business days prior to the expiry of your current membership period.
1.10 Any donation amount that you add to your membership payment, will not form part of this Direct Debit Agreement. This will be considered a one-off donation and will not be debited on any ongoing basis.
2. Amendments by us
2.1 We may vary any details of this agreement or a Direct Debit Request at any time by giving you at least fourteen (14) days written notice.
2.2 Membership fees may change from year to year. If there is a change to your annual membership fee, we will provide you with at least fourteen (14) days written notice before your debit day.
3. Amendments by you
3.1 You may change or cancel your Direct Debit arrangement directly at any time by contacting us at: membership@diabetesvic.org.au or by telephoning us on 1300 437 386 during business hours.
4. Disputes
4.1 If you believe there has been an error in debiting your account, you should notify us directly on 1300 437 386 or email us at membership@diabetesvic.org.au
4.2 If we conclude as a result of our investigations that your account has been incorrectly debited, we will adjust your account accordingly. We will also notify you in writing of the amount by which your account has been adjusted.
4.3 If we conclude as a result of our investigations that your account has not been incorrectly debited, we will respond to your query by providing you with reasons and any evidence for this finding in writing.
5. Confidentiality
5.1 We will keep any information (including your account details) in your Direct Debit Request confidential. We will make reasonable efforts to keep any such information that we have about you secure and to ensure that any of our employees or agents who have access to information about you do not make any unauthorised use, modification, reproduction or disclosure of that information.
5.2 We will only disclose information that we have about you:
a. to the extent specifically required by law; or
b. for the purposes of this agreement (including disclosing information in connection with any query or claim).
