Free Blood Glucose Meter Promotion Terms and Conditions 2025

  • This offer is promoted by Diabetes Victoria (ABN 71 005 239 510), Suite G01, 15-31 Pelham Street, Carlton VIC 3053.
  • The free blood glucose meters will be supplied and dispatched by Pharmaco (Australia) Ltd (ABN 89 113 383 501) of Level 9, Tower A, Zenith Towers, 821 Pacific Highway, Chatswood, NSW 2067.
  • A total of 400 free blood glucose meters are available during this promotion.
  • The promoter reserves the right to increase the total quantity of free meters available during the promotional period at its discretion.
  • The offer opens at 9:00am AEST on Monday 4 August 2025 and closes at 5:00pm AEST on Friday 26 September 2025, or earlier if all available meters are claimed.
  • No further free meter applications will be accepted after the closing date or once supplies are exhausted.
  • To be eligible, you must meet all of the following criteria:
    • Pay a full Diabetes Victoria membership fee during the promotional period:
      • $35 (concession card holders)
      • $57 (non-concession card holders)
      • $58 (family or health professional member)
    • Complete the free meter application form.
    • Be aged 18 years or older as of 4 August 2025.
    • Reside in the state of Victoria.
    • Be registered with the National Diabetes Services Scheme.
    • Have been advised by a healthcare professional to undertake blood glucose monitoring.
    • You agree to consult your healthcare professional to confirm the product is suitable for you.
    • You agree to consult your healthcare professional before making any changes to your diabetes management.
    • You agree to Diabetes Victoria’s Privacy Policy: diabetesvic.org.au/privacy
    • You consent to your name and postal address being shared with Pharmaco for the sole purpose of mailing the free meter.
  • There is a limit of one free meter per eligible member.
  • Submission of your free meter application constitutes acceptance of these terms and conditions.
  • Applications that do not comply with these terms and conditions will be deemed invalid.
  • This promotion is not available to holders of free Diabetes Victoria memberships.
  • Diabetes Victoria is not responsible for free meters that are not received.
  • Pharmaco (Australia) Ltd is responsible for the supply of the free meters.
  • Your personal information will only be shared with Pharmaco for the purpose of meter fulfilment and will not be used for marketing unless you provide further consent.
  • Pharmaco (Australia) Ltd will aim to dispatch the meter within 3 business days of application confirmation.
  • Delivery timeframes may vary based on demand and postal condition.
  • All CareSens meters are subject to the standard warranty provided by Pharmaco (Australia) Ltd.
  • Diabetes Victoria is not responsible for product defects, damage, or failure.
  • Please contact Pharmaco (Australia) Ltd directly regarding product issues.
  • Participation in this promotion does not constitute endorsement by Diabetes Victoria of Pharmaco or any CareSens products, services, or information.
  • Pharmaco (Australia) Ltd retains responsibility for its products, services, trademarks and other materials.
