A New Era in Diabetes Care: Why Diabetes Care Plus is More Than a Name Change

The dedicated team of diabetes specialists have supported better health outcomes through face-to-face appointments as well as telehealth sessions, servicing metro, rural and remote areas of Victoria.

Today, as we launch Diabetes Care Plus, we are fulfilling our commitment with a broader vision. 

What does it mean for you?

If you already receive care through The Diabetes Victoria Clinic, nothing changes. You’ll still receive the same personalised care from familiar faces under the new banner. Your appointments, records, and contact details remain the same.

Change is the opportunity

We have now expanded access through our telehealth services. We aim to bring expert diabetes care to people who need it in remote, regional, and underserved communities. There will be more educational and self-management support programs, online resources, and group education offerings to help you live confidently and well with (or at risk of) diabetes.

Professional development & training

We’ll offer structured education and online modules to help professionals stay current in diabetes care.

This will extend to:

  • Disability support workers
  • Aged Care support workers
  • Educations workers
  • Health professionals
