The annual cycle of care is a well thought out, coordinated plan that includes a series of checks and assessments.
These include:
- HbA1c levels
- Blood pressure
- Cholesterol
- Kidney function
- Eye health
- Foot health, and more.
These checks help detect early signs of complications and keep treatment plans on track.
Breaking the routine
The annual cycle of care is the guide, it’s not rule. Remember, if something doesn’t feel right, or if something changes dramatically, you don’t have to wait for your next scheduled check.
Your health doesn’t follow a calendar – and neither should your care. If you have concerns, please see your health professional immediately.
How a ‘Team Care Arrangement’ works for you
In Victoria, if you’re living with diabetes, you may be eligible for a Medicare-funded Team Care Arrangement (TCA). This is a coordinated care plan developed by your GP, often with the help of a practice nurse. This will ensure you receive comprehensive, ongoing support with costs subsidised.
With a TCA, you can access up to five subsidised visits per year to allied health professionals such as:
- Diabetes educators
- Dietitians
- Podiatrists
- Exercise physiologists
- Psychologists
These visits are designed to complement your Annual Cycle of Care by helping you manage your condition day-to-day and work toward your personal health goals.
