Diabetes Paediatric to Adult Transition in Healthcare (PATH) Project

Have you or someone you care for transitioned from paediatric to adult diabetes care?

The Diabetes PATH project, led by the Monash Centre for Health Research and Implementation (MCHRI), is exploring what success and best practice look like when moving from paediatric to adult diabetes care.

Adolescents and young adults with type 1 diabetes often face challenges in self-management and continuity of care during this critical phase. This project seeks to better understand and support their experiences.

By sharing your experiences through an online survey (15-20 minutes) and an optional focus group or interview (45-60 minutes), you can help improve support for young people with type 1 diabetes as they navigate this important stage of life.

Who can take part:

✅ Young adults aged 18–35 living with type 1 diabetes
✅ Parents/carers of adolescents and young adults with diabetes aged 14–35
✅ Clinicians who work with adolescents and young adults with diabetes
